Another dropped object accident

A lifting line was broken. What would be an aftermath of this ultimately leading to partial capsizing? No clue whether there was a fallen object or the loosened one hitting the hull to resut in a hull puncture and flooding. I would argue this is how the multi-stage bowtie analysis can give better description of diverse mechanisms of accidents.
PS. There was a follow-up report from Saipem that the accident happened during a test and the barge being lifted was released onto the sea at the forward starboard side. The broken line had been holding the lifting block that in turn holds a dozen lines connected to the barge. My question: was there only one line broken to release the block or more than one line failed? Is it designed to allow failure of one line to drop a lifting object?
